This form may be used to request a copy of a previously-created Educational Plan. Do not use this form to request a counseling appointment to develop a new Educational Plan. If you need a new educational plan, please call the Counseling Department at 909-652-6200.

Please make sure to fill out this form completely.
Most fields require input to ensure we can find your Educational Plan.
Your results will be available for pick up from the Rancho Campus Counseling Department front desk after 48 business hours.
You must display a photo ID to pick up the Educational Plan.

The Counseling Department does not guarantee that we will be able to locate your Educational Plan. If we cannot find it, you will need to schedule an appointment with a counselor and have a new plan created.
